M., Rajnish kumar, Shailender Mohan<\/p>\n<div style=\"text-align: left;\">\u00a0<span class=\"cm_word\" style=\"border-bottom: 1px solid #0000FF !important;text-decoration:underline !important;color:#0000FF !important\">Diabetes mellitus<\/span> is <span class=\"cm_word\" style=\"border-bottom: 1px solid #0000FF !important;text-decoration:underline !important;color:#0000FF !important\">a serious pathologic<\/span>\u00a0<span style=\"font-size: 10pt;\">condition which is responsible for major<\/span><\/div>\n<div style=\"text-align: left;\"><span class=\"cm_word\" style=\"border-bottom: 1px solid #0000FF !important;text-decoration:underline !important;color:#0000FF !important\">healthcare problems<\/span> worldwide and costing\u00a0<span style=\"font-size: 10pt;\">billions of dollars annually. Insulin replacement<\/span><\/div>\n<div style=\"text-align: left;\">therapy has been used in the clinical\u00a0<span style=\"font-size: 10pt;\">management of diabetes mellitus for more than<\/span><\/div>\n<div style=\"text-align: left;\">84 years.\u00a0<span style=\"font-size: 10pt;\"><span class=\"cm_word\" style=\"border-bottom: 1px solid #0000FF !important;text-decoration:underline !important;color:#0000FF !important\">Insulin<\/span> has remained indispensable in the\u00a0<\/span><span style=\"font-size: 10pt;\">management of diabetes mellitus since its\u00a0<\/span><span style=\"font-size: 10pt;\">discovery in 1921. Comparatively, a large\u00a0<\/span><span style=\"font-size: 10pt;\">percentage of world population is affected by<\/span><\/div>\n<div style=\"text-align: left;\">diabetes mellitus, out of which approximately 5-<span style=\"font-size: 10pt;\">10% with type 1 diabetes while the remaining\u00a0<\/span><span style=\"font-size: 10pt;\">90% with type 2.\u00a0<\/span><span style=\"font-size: 10pt;\"><span class=\"cm_word\" style=\"border-bottom: 1px solid #0000FF !important;text-decoration:underline !important;color:#0000FF !important\">The present mode<\/span> of insulin administration is\u00a0<\/span><span style=\"font-size: 10pt;\">by the subcutaneous route through which insulin\u00a0<\/span><span style=\"font-size: 10pt;\">is introduced into the body in a non-physiological<\/span><\/div>\n<div style=\"text-align: left;\">manner having <span class=\"cm_word\" style=\"border-bottom: 1px solid #0000FF !important;text-decoration:underline !important;color:#0000FF !important\">many challenges<\/span>. Hence novel\u00a0<span style=\"font-size: 10pt;\">approaches for insulin delivery are being<\/span><\/div>\n<div style=\"text-align: left;\">explored.\u00a0<span style=\"font-size: 10pt;\">Challenges that have adverse effect on oral\u00a0<\/span><span style=\"font-size: 10pt;\">route of insulin administration mainly includes:\u00a0<\/span><span style=\"font-size: 10pt;\">rapid <span class=\"cm_word\" style=\"border-bottom: 1px solid #0000FF !important;text-decoration:underline !important;color:#0000FF !important\">enzymatic degradation<\/span> in the stomach,\u00a0<\/span><span style=\"font-size: 10pt;\">inactivation and digestion by proteolytic enzymes\u00a0<\/span><span style=\"font-size: 10pt;\">in the intestinal lumen and poor permeability\u00a0<\/span><span style=\"font-size: 10pt;\">across intestinal epithelium because of its high\u00a0<\/span><span style=\"font-size: 10pt;\">molecular weight and lack of lipophilicity.\u00a0<\/span><span style=\"font-size: 10pt;\">Approaches such as liposome, micro\u00a0<\/span><span style=\"font-size: 10pt;\">emulsions, nanocubicle, insulin chewing gum and\u00a0<\/span><span style=\"font-size: 10pt;\">so forth have been prepared to ensure the oral\u00a0<\/span><span style=\"font-size: 10pt;\">delivery of insulin. Attempts have been made to\u00a0<\/span><span style=\"font-size: 10pt;\">achieve oral insulin delivery using various\u00a0<\/span><span style=\"font-size: 10pt;\">systems.
